The Cummins LiveWell Center has launched My Premise Health, a member portal where you make your appointments and access information about your healthcare. This is provided through Premise Health who operates the LiveWell Center for Cummins.
My Premise Health connects you with your healthcare experience by providing convenient virtual visits, access to your providers, health records, visit history, test results, seamless sharing of information with community providers and more. It is a secure tool that puts your health information in your hands.
Active Cummins employee sign-up:
- Visit cumminslivewell.com and click on the schedule an appointment button at the top.
You can also download the mobile app from the App Store or Get it on Google Play.
Spouses and dependents sign-up:
- Email [email protected] and provide a phone number for a return call. Available Monday – Friday, 6 a.m. CST – 7 p.m. CST., OR
- Complete registration at the time of your next appointment, OR
- Visit or call the LiveWell Center at 812. 748. 3412
Once you have created an account online, you can download and log in to the mobile app.
